En 2014, l'Alerte gymnastique s'est doté d'un nouveau logo. Vous le retrouverez bien évidemment sur l'ensemble des documents que nous vous transmettons, mais également très bientôt dans notre "boutique" du club.

Vous devez remplir le questionnaire de santé suivant pour l'adhérent : https://www.formulaires.service-public.fr/gf/cerfa_15699.do Selon vos réponses au questionnaire, vous devrez fournir l'un des documents suivants pour obtenir votre licence : - Attestation de renseignement du questionnaire - Certificat médical attestant de l'absence de contre-indication à la pratique du sport concernée. Le certificat doit dater de moins de 6 mois au jour de la demande de licence. Pour la pratique compétitive, il est indispensable de faire établir le certificat médical avec la mention "non contre-indication à la pratique de la gymnastique EN COMPETITION". joindre un fichier "CM_nom_prenom.pdf" de 2MB max.
